“I, the undersigned, authorize Revive Therapeutic Massages to securely store my credit card information for the sole purpose of applying a cancellation or no-show fee. No charges will be made at the time of booking. Charges will only apply if I cancel with less than 24 hours’ notice or do not show up to the appointment. The fee will be equal to 50 % of the scheduled services price.
We accept Mastercard, Visa, check and cash. Payment by check or cash whenever possible is appreciated.
